Transferring In?
Welcome!! We are excited to have you join our All Star Team!
1) Please read our "Are we a Good Fit?" to determine if we are a good fit, and if we are go straight to following the steps below.
2) Another great option is to come in the office to meet us by requesting a "Meet the Docs" or see our virtual "Meet the Docs".
What WE NEED TO GET STARTED :
We do require all forms to be completed before we can schedule your first appointment. These can all be emailed to us directly using our HIPPA secure email:
patientcare@aspeds.com.
- New family registration forms (one for family)
- AND Patient medical history form (one per child)
- Immunization records, last well child visit report, and other significant medical records.
- Copy of your insurance card (front and back side)
* We are only able to accept paper or digital files, but not accept disc/cds containing medical records.
Transferring out?
We are sad to see you go. We know there are many reasons people have to leave at times and its helpful for us to know. Please let us know whether its due to age, insurance changes or just not a right fit. We strive to do our best and welcome feedback.
Patients tranferring out of our practice (due to aging out or transferring practices) can print their medial records for free through the patient portal or pay a $20.00 fee per child and will need to fill out this Medical Release Form and email it to patientcare@aspeds.com.
